Guide for practices: assign, collect and evaluate questionnaires
The HBImed Questionnaire App is a web-based platform for administering, collecting and evaluating clinical questionnaires. Clinicians create patients, assign questionnaire packages, and send the patient a link. Patients complete the questionnaires in their preferred language; the clinician then receives the results as an on-screen report, as raw data, or as a Report-Generator (RG) Excel file for the downstream MATLAB analysis.
The app runs at https://hbimed-fragebogen.gtsg.ch.
The app uses a three-role model:

Key visibility rules:
New users receive an initial password and are prompted to set their own on first login.

Each questionnaire’s title, instructions, item texts and answer labels can be translated per language. Every translation has a status:

Only translations with status freigegeben are shown in the patient portal. Newly imported or machine-assisted translations start as entwurf and must be reviewed (for clinically sensitive or non-Latin-script languages, additionally by a native speaker with clinical background) before being released.
All questionnaires active in the system. This list is generated automatically from the database and is therefore always current. “RG” = wired for the Excel export to the MATLAB Report Generator. The HBI numbering follows the internal coding scheme.
